In return for being permitted to participate in KINGDOM HOOPS LLC. At Paradise High School located at 338 School House Road, Paradise, TX 76073 and understanding that there are certain risks in connection with such activity, and intending to be legally bound hereby, the undersigned, for himself/herself, assigns and legal representatives, hereby expressly agrees to: Release, waive, discharge, and agree not to sue the Board of KINGDOM HOOPS LLC and all its successors, assigns, affiliates, officers, directors, employees, and agents ("KINGDOM HOOPS LLC") from all manner of actions and causes of action, suits, debts, accounts, judgments, claims, and demands whatsoever in law or equity, and attorney's fees, including all claims arising out of any incidents involving personal injury in any way by reason of participation in the Program; Assume any and all risks arising from his/her participation in the voluntary activity named above, including without limitation, the risks of bodily injury or property damage, the unavailability of emergency medical care, or the negligent or deliberate act of another person. Indemnify, defend and hold the Board of KINGDOM HOOPS LLC, its directors, officers, employees, and agents harmless from any and all claims, causes of action, damages, judgments, costs, or expenses that arise out of or relate to the negligent or intentional act or omission of the undersigned. Each of the undersigned expressly acknowledges that he/she has read and understands this Agreement and Release and signs it freely and voluntarily. My son/daughter, is participating in KINGDOM HOOPS LLC summer camp at Paradise High School located at 338 School House Road, Paradise, TX 76073 on June 26-28 2023. I have carefully read, understand, and have signed the KINGDOM HOOPS LLC Summer Camp Participant Agreement. I also have carefully read and understand the terms of this permission form and have had the opportunity to ask any questions I may have. Thereby I give my permission for my son/daughter to participate in the program, and any and all of its activities, and agree to release, indemnify, and hold harmless KINGDOM HOOPS LLC from and against any claim which I or my son/daughter or any other person may have for any losses, damages or injuries arising out of or in connection with my child's participation in the summer program. It is agreed that my child's participation is adequate consideration. I understand that photographs, video recordings, or audio recordings may be taken of me during my participation in this Summer Camp by employees, and volunteers of KINGDOM HOOPS LLC shall be used in connection with the KINGDOM HOOPS LLC dissemination of information by its public service programs to the general public. I authorize the KINGDOM HOOPS LLC to copy, exhibit, publish or distribute any and all such images and audio of me or where in I appear, for purposes of publicizing KINGDOM HOOPS LLC programs or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written copy, wherein my likeness appears.
